Diverse Mind Care offers long-term online care and medication management for a wide range of mental health conditions listed. First-time patients must book an initial evaluation appointment upon booking. You will receive our intake forms via email. All intake forms must be completed within 24 hours to confirm the appointment.

In addition, you will be able to access the Psychiatric Mental Health Nurse Practitioner through telehealth visits. The nurse practitioner will determine your treatment plan and prescribe medication as appropriate. During your monthly video appointment, the Psychiatric mental health nurse practitioner will track your progress with the recommended treatment plan and adjust your medication as needed.

Initial Evaluation - 60 minutes starting - available the same day with our Psychiatric mental health nurse practitioners and includes a supportive psychotherapy session to discuss how you are feeling and managing your medications.

Follow-up appointments - 45 minutes - available the same day with our Psychiatric mental health nurse practitioners and include supportive psychotherapy sessions to discuss how you feel and manage your medications.

Diverse Mind care is an in-network with Cigna. 

If you do not have health insurance, we will provide you with a receipt to submit to your insurance and can assist you in the process of submission to your insurance Company.

The Psychiatrist Mental Health Nurse Practitioner will need to confirm your identity and date of birth before you can be treated via telemedicine. Diverse Mind Care requires an identifiable photo of your face and an identifiable photo ID. This must include:

First Name, Last Name, Date of birth, Expiration date (for legal purposes your ID cannot be expired), Unobstructed photo of your face.
